Excerpt:  Illustration of N6324C about to collide with N6902C The 1956 Grand Canyon mid-air collision occurred on Saturday June 30, 1956 at 10:30 AM local time when a United Airlines passenger airliner struck a Trans World Airlines (TWA) airliner over the Grand Canyon in Arizona , resulting in the crash of both planes and 128 fatalities. It was, at the time, the deadliest aviation disaster in history, and would be a catalyst for sweeping changes in the regulation of flight operations over the United States. The flights United Airlines Flight 718 , a Douglas DC-7 Mainliner named Mainliner Vancouver , and piloted by Captain Robert Shirley and First Officer Robert Harms, departed Los Angeles International Airport at 9:04 AM with 53 passengers and five crew members aboard (including two flight attendants ), and headed to Chicago 's Midway Airport . Climbing to an authorized altitude of 21,000 feet, Captain Shirley flew under instrument flight rules (IFR) in controlled airspace as far as Palm Springs, California . At Palm Springs, he changed course to a heading of 046 degrees magnetic , which was in the direction of St. Joseph, Missouri . The DC-7, although still operating under IFR, was now "off airways " that is, flying in uncontrolled airspace. TWA Flight 2 , a Lockheed Super Constellation named Star of the Seine , with pilot Jack Gandy and copilot James Ritner in the cockpit, departed Los Angeles at 9:01 AM with 64 passengers and six crew members (including two flight attendants and an off-duty flight engineer), and headed to Kansas City Downtown Airport , 31 ...</p>

<p>"HUMAN FACTORS IN MULTI-CREW FLIGHT OPERATIONS"  co-written by Harry and Linda Orlady - Ashgate ISBN 0-291-39839-1 - Published in 1999
<p>A father and daughter team! It is rare enough to have a flying father and a flying daughter in the real world, but sharing the same passion so intensely and to write a book together on an all-encompassing aviation Bible, (yes, this book is a Bible for aviators!) is truly unprecedented!
<p>When I said this book covers virtually all topics concerning flight operations related topics, I meant just that. The most important thing is; this book is highly readable, rich in reference materials and data and yet "gripping" to read! A rare gem in terms of human factor topics. I have read other CRM or human factors books by other more renowned and authoritative people but none as good as the Orladys, probably with the exception of Tony Kern, who is also just as good a writer on aviation safety.
<p>Okay, the book covers the brief history of air transport, the industry and its safety record,, a brief history of human factor and its development in aviation,, the physical environment and the physiology of flight, as well as those magnificent flying machines and their internal  environment - sort of like a trip down to aviation memory lane.
<p>It also discussed the social environment, basic communication, documentation - including checklists and information management. It went on to discuss on the Man's limitations, human errors, and information processing. Nothing is left unturned, the Orladys went on to talk about workload, automation, situation awareness and operating in today's environment. Of course, they did not miss out on crew resource management and the team approach.
<p>Fatigue and stress were covered in depth, plus fitness to fly, even selection and training of pilots! Most interestingly is the coverage on the challenging roles of the flight attendants, this shift in focus of our cabin colleagues was most insightful.
<p>I loved the chapter on non-punitive incident reporting.. the CHIRP and ASRS were great success stories in UK and USA respectively, I fervently hope that SIA will follow suit in our pursuit of excellence in aviation flight safety.
<p>Another eye-opener chapter is "some ramifications of accident analysis", this is the first time I heard of the "Stop Rule" phenomenon in flight safety investigation - find it out yourself what it means.
<p>In the last few chapters, the Orladys talked about the worldwide safety challenge in the near future, the current safety problems and the future of air transport too.
<p>This is not a book to be missed by any pilot who wish to enrich himself, you may get this book from AMAZON dot com. Hardcover costs US9.95! and papaerback costs US.95!

<p><P>Bravery in the face of unimaginable terror prevented greater tragedy on September 11, 2001, when a group of passengers overpowered the hijackers of United Airlines Flight 93. Their heroism and sacrifice inspired us all. One of these passengers was Mark Bingham, a living-out-loud, gregarious, gay man. But who was Mark Bingham really? What was it about this man that caused his friends to unanimously say that he must have been one of the men who rushed the hijackers? The Advocate’s senior news editor Jon Barrett interviewed those who knew him best, starting with the mother who instilled in him the belief that he could be anything, to the friends, lovers, business associates, and rugby teammates who complete the picture of a man determined to never take second place. This is his story, told to remind all Americans that heroism knows no sexuality.</P><P><I>Jon Barrett </I>is the senior news editor for <I>The Advocate </I>magazine, and the author of the cover story on Mark Bingham in <I>The Advocate </I>upon which this book is based. <p>In the aftermath of 9/11, several news articles mentioned that Mark Bingham was gay and a Republican. As a gay Republican myself, this naturally interested me. However, according to this biography, he was actually a Libertarian, though like most Libertarians he agreed with the GOP on many issues and on at least one occasion he worked for a Republican's campaign.</p>
<p>What's really striking about this biography is how very normal Mark Bingham was. He was nicer than a lot of people, and he did show a protective instinct all of his life; once he attacked a mugger to protect his friends from him. But aside from this, he was a fairly ordinary young man with career successes and failures behind him, working and having fun with his friends and wondering if he was doing the right thing with his life and if he was ever going to find a life partner.</p>
<p>It was also a little amusing that the biographer tried to gloss over one of the few qualities in Mark Bingham that would be likely to offend some: he was a "bear" and like some conventionally masculine gay men, effeminate gay men bothered him. Barrett mentions this as briefly as he can and then emphasizes the more tolerant comments Bingham made about effeminate gay men, trying to imply that Bingham knew his distaste for them was wrong and was striving to overcome it. Maybe that's true, but I could see that Barrett was concerned that this bit of personal taste might prejudice gay readers against him.</p>
<p>All in all, it's a good and balanced study of an ordinary man who became a hero because the opportunity sought him out. It's inspiring to know that such an ordinary man can be so heroic.</p>
